Removed all patch kit headers, sccsid and rcsid strings, put $Id$ in, some

Removed all patch kit headers, sccsid and rcsid strings, put $Id$ in, some
minor cleanup.  Added $Id$ to files that did not have any version info, etc

NOTE:  This stuff is really old, and should be removed, but it is left here
for reference only.  It does not even compile any more!
